Global Exposure and Growth at the Island Meeting: Sightsavers India Fellowship Opportunity

Dr Aniza Nagpal, November 2024

As part of my Sightsavers India Fellowship Programme, I had the incredible opportunity to attend the 32nd Annual Ophthalmology Congress in Colombo, Sri Lanka, from the 11th to the 15th of September, 2024. This international event brought together eye care professionals from around the globe to discuss the latest advancements and challenges in the field of ophthalmology. It was an enriching experience that allowed me to engage with the most current trends in eye care.

Learning about Phacoemulsification Technology

One of the key sessions I attended was on the latest advancements in phacoemulsification technology. I got to witness the performance of new, cost-effective phaco machines, including the Hummingbird by Aurolab and Cube Alpha by Nidek. I even had the opportunity to perform phacoemulsification on wax eyes using these machines, which gave me hands-on experience in understanding their functionality and precision. The potential of these machines to revolutionize cataract surgery, especially with their cost-effectiveness, is immense, and I left feeling confident about their application for beginner cataract surgeons.

Global Trends in Eye Health

I also attended an insightful discussion on the current global scenario of eye healthcare. The session emphasized the urgent need to increase access to quality eye care in underserved regions, tackle cataract blindness, and address the growing challenges posed by ageing populations worldwide. It was highlighted that universal eye care could be transformative for countries like Sri Lanka, India, and other SAARC nations.

We also learned about the introduction of advanced technologies in Sri Lanka, which are now gaining popularity. For example, iStent and selective laser trabeculoplasty (SLT), previously less common, are now being actively promoted by doctors like Dr. Dilruwani Aryasingha. Additionally, Dr Imalka Fonseka and other experts have been making significant strides in improving the treatment options for Sri Lankan glaucoma patients who were not suitable candidates for conventional topical or surgical management. Under Dr. Aryasingha’s leadership, studies are being conducted to explore the long-term benefits of SLT, its potential need for repeated procedures, and its role as a first-line treatment for mild to moderate glaucoma cases.

Interaction with Renowned Ophthalmologists

Networking with world-class ophthalmologists was a highlight of the congress. I had the privilege of interacting with renowned experts like Dr Ronald Yeoh, Dr Dushyantha Wariyapola, Dr Charith Fonseka, Dr K.A. Salvin, Dr Kapila Bandutileke, Dr K.R. Dayawansa, and Dr Harshul Tak.

Dr. Yeoh emphasized using the Barrett formula for accurate IOL power calculations and the importance of averaging K-values from different equipment to reduce errors. The sessions focused on key topics such as dry eye assessments, proper equipment calibration, cross-checking parameters with multiple modalities, and understanding surgically induced astigmatism (SIA) to improve postoperative outcomes.

Engaging with these experts provided valuable insights into advanced surgical techniques and approaches to handling complex cases, giving me a global perspective on tackling region-specific challenges while optimizing resources.

Technological Advancements and Future Directions

The technological innovations showcased during the sessions were truly inspiring. From advancements in phacoemulsification machines to global efforts aimed at reducing preventable blindness, I felt energized by the direction ophthalmology is taking. Collaborative efforts on the part of both developed and developing nations were key takeaways for me from the 2024 Congress. The compact and user-friendly design of machines like the Hummingbird will be especially beneficial for cataract surgeries in resource-limited settings, while the precision of the Cube Alpha will enhance outcomes in complex cases.

Insights and Recommendations from the Experience

The management protocol for normal-tension glaucoma, as outlined by Dr. Imalka Fonseka, the head of Glaucoma in Sri Lanka, gave me valuable insights into handling such cases in my practice. His detailed summary of treatment options and long-term care strategies was particularly helpful for managing this condition effectively.

For anyone planning to attend future congresses, I highly recommend focusing not just on the sessions but also on networking with fellow professionals. Engaging with the innovations on display is key, and don’t miss out on product exhibitions, as they provide valuable hands-on experience with the latest technology.

Overall, this congress was a truly enriching experience. I am incredibly grateful to the Sightsavers team for offering me this opportunity, which will undoubtedly help me enhance my practice and make a meaningful impact on the patients I care for in the future.
